All medication to be clearly labelled and explained, and given directly to the assigned person in charge of assisting children with their medications (enquire at registration point on arrival)

I hereby give consent for my child to participate fully in MAGNIFIED! Holiday Bible Club at the Honeyridge Baptist Church. I fully understand and accept that any activities shall be undertaken at my child's own risk and I undertake on behalf of myself, my Executors, my spouse and my child aforesaid to indemnify, hold harmless and absolve Honeyridge Baptist Church, the Pastors, Elders, employees and the youth leadership against and from any or all such claims whatsoever that may arise or be made in connection with any loss of life of my child, loss of or damage to the property or injury to the person of my child in the course of the activities, in the knowledge that the leaders will nevertheless take all reasonable precautions for the safety and welfare of my child.

Additionally, I understand that my child may not leave the property to be collected outside by third parties without prior arangement with senior organisers of the  Holiday Bible Club.

In addition, I, as parent/legal guardian of the children registered in this form hereby consent to Honeyridge Baptist Church processing the personal information as provided herein in terms of the Protection of Personal Information Act 4 of 2013 for the purposes of planning for and running the holiday club which may include, but is not limited to storing the information on a database and sending communication to me concerning the holiday club and church related activities which may be applicable to my children.

What we Believe

With regards to special needs, please note that as far as possible we can accommodate children with special needs that are unable, or struggle to integrate in group environments (small groups & games) as well as crowded spaces (such as hall time for worship & war cries). At this point in time we do not have specially trained personel that can accommodate particular specific needs, especially those that require more specific attention or that require one on one assistance.
